Hailey Crean Nutrition, LLC, offers the Virtual Diabetes Education Program® to people in Ohio who want to learn to manage their diabetes. We provide nutrition support and diabetes education using telehealth.

As one of the few completely virtual Association of Diabetes Care and Education Specialists (ADCES) accredited Diabetes Self-Management Education and Support (DSMES) programs for treating diabetes and conditions of metabolic syndrome in the country, we can give our patients the evidence-based benefits of DSMES while adding the convenience of virtual programs.

We work with Healthie to bring you a HIPAA-compliant telehealth and messaging system, letting you have appointments with a virtual diabetes dietitian from your tablet, phone, laptop, or any other device in a secure and private manner.

Diabetes Self-Management Education and Support (DSMES)

The Virtual Diabetes Education Program® offers individual and group DSMES sessions for patients in Ohio who have:

  • Type 1 diabetes
  • Type 2 diabetes
  • Gestational diabetes
  • Prediabetes
  • Latent autoimmune diabetes in adults
  • Maturity onset diabetes of the young

Our evidence-based online diabetes classes allow patients to improve their health, with studies showing that DSMES can help lower blood pressure, cholesterol levels, and A1C levels. Since these are diabetes markers, reducing them can reduce the risk of diabetes complications, including hospitalizations.

By signing up for the Virtual Diabetes Education Program®, you will get a person-centered approach to diabetes and cardiometabolic care. In our diabetes education classes, you will learn all about the ADCES seven self-care behaviors for the management of diabetes. These behaviors include:

  • Healthy eating
  • Problem-solving
  • Healthy coping
  • Being active
  • Taking medications
  • Reducing risk
  • Monitoring

We individualize the program for each patient, focusing on the areas that cause the most difficulties for each person.

Since we are an accredited and insurance-reimbursable program, Medicare recipients and those who have most other private insurance plans have coverage for DSMES.

Online Diabetes Support Group

We offer free monthly support groups that meet for 45 minutes using secure Zoom spaces that are HIPAA compliant. Struggling with diabetes on your own, whether you have a new diagnosis or have been dealing with diabetes for years, can isolate you.

How to Start

If you have diabetes, there are four important moments when you need to see a Certified Diabetes Care and Education Specialist (CDCES):

  • When first diagnosed with diabetes
  • When not meeting healthcare goals
  • When there are new complications
  • When there is a change in your care

To begin our online diabetes education program, you can schedule a 20-minute complimentary session with an online diabetes dietitian. During this session, you can share your long-term health goals, learn more about the Hailey Crean Nutrition, LLC, approach, and ask any questions you may have.

If you feel our program is a good fit, we can schedule your 60-minute comprehensive initial assessment. Together, we will review your exercise and eating habits, any past diets you have followed, stress management, and more.

Once we have an accurate idea of your lifestyle and individual challenges, we can create a customized education plan. This will include meal planning support.
