About Us

Hailey Crean Nutrition, LLC is a telehealth practice that is ADCES accredited. We specialize in virtual diabetes management education and providing support to those who have this condition. We also work with patients who have conditions of metabolic syndrome.

Our online diabetes education classes are entirely virtual and designed to give you the tools you need to work through a new or changing diagnosis. You can receive vital information from the comfort of your own home and gain access to additional diabetes resources.

You can also speak to other people living with diabetes through our online support group, which allows you to connect with people in similar situations.

Our Virtual Diabetes Education Program

The Virtual Diabetes Education Program® at Hailey Crean Nutrition is a comprehensive program that includes a variety of features, including a virtual diabetes dietitian and online support meetings. You’ll benefit from the following components of our Iowa diabetes program:

Registered Dietitian for Medical Nutrition Therapy

When you begin our virtual diabetes program, you will receive one-on-one sessions with a registered dietician for medical nutrition therapy (MNT).

Our registered dieticians specialize in prediabetes, diabetes, high cholesterol, high blood pressure (hypertension), and high triglycerides. We will give you detailed guidance on how to manage your condition, assess your cholesterol and triglyceride levels, and keep your blood pressure low.

Diabetes Self-Management Education and Support

Also included in the virtual diabetes program are our Diabetes Self Management Education and Support sessions (DSMES). We offer educational resources and online support for people living with prediabetes, type 1 diabetes, type 2 diabetes, gestational diabetes, LADA, or MODY.

There are four primary times you should seek the assistance of a certified diabetes care and education specialist (CDCES):

  • When you are newly diagnosed with diabetes
  • When you are not meeting healthcare goals
  • If complicating factors arise in your condition
  • When you are transitioning to a new medical team

In most cases, DSMES is covered for Medicare recipients and the majority of private insurance plans. Before starting our virtual program, you should check with your insurance provider to confirm that your CDCES will be covered.

Education Sessions

The primary purpose of our online diabetes classes is to provide further education on living with diabetes. We offer individualized education resources that teach you how to manage your disease and live with any associated symptoms.

Our program centers around seven self-care behaviors that can guide you through life with diabetes. They include:

  1. Healthy eating
  2. Being active
  3. Healthy coping
  4. Taking medications
  5. Reducing risk
  6. Problem-solving
  7. Blood sugar monitoring

Your online diabetes dietician will use a person-focused approach that takes your unique needs into consideration. We will work through your specific concerns and help you develop a personalized way to navigate your diabetes.

Online Diabetes Support Group

Another feature of our Iowa online diabetes programs is the virtual support group. Members have access to a monthly support group where they can share their concerns and personal stories with others living with diabetes. These support group sessions are about 45 minutes in length and completely free of charge. They are also HIPAA compliant and take place over a secure Zoom session.
