About Our Team
Hailey Crean Nutrition, LLC is an ADCES-accredited telehealth practice that specializes in diabetes management education. We provide virtual support to people who have recently been diagnosed with diabetes or those who are going through a transition in their condition. Hailey Crean also works with people who have conditions of metabolic syndrome.
We aim to make living with diabetes easier by offering completely virtual support groups, online courses, and more. This allows you to access the information you need in order to stop the progression of your diabetes or manage your existing symptoms.

The biggest benefit of our Tennessee virtual diabetes program is the education it provides. When you sign up, you will have access to exclusive online diabetes education classes with an individualized approach. Our courses center around a seven-part self-care system that includes:

Virtual Diabetes Support Meetings
One of the best features of our virtual diabetes program is the online support group. Our program offers monthly meetings for people living with diabetes to talk to one another and share their experiences with the condition.
These 45-minute online sessions allow you to connect with others and gain more insight into this condition. All of our support meetings are completely secure and HIPAA-compliant.
Diabetes Self-Management Education and Support
A quintessential aspect of our online program is the Diabetes Self Management Education and Support sessions. Also known as DSMES, these sessions provide virtual support to people with any of the following conditions:
- Type 1 diabetes
- Type 2 diabetes
- Gestational diabetes
There are four main times you should have a DSMES with a certified diabetes care and education specialist (CDCES). The first is when you are newly diagnosed with diabetes. The next is any time you are not meeting your healthcare goals. Another is if any new complicating factors arise, such as another medical concern. The final is when you are moving or transitioning to a new care team.
If you are worried about the cost of these services, always check with your insurance carrier prior to starting the virtual program. In most cases, private health insurance and Medicare will cover DSMES.
Online Diabetes Dietician
With our online diabetes programs, you will be assigned a telehealth diabetes dietitian who can guide you through your diagnosis. All of our dieticians specialize in diabetes, prediabetes, high cholesterol, hypertension, and triglyceride management.
Your registered dietician for medical nutrition therapy (MNT) will help you manage your cholesterol and triglyceride levels and give you the information you need to avoid high blood sugar or severe diabetes symptoms.
