This 60-minute session will focus on an assessment of all nutrition-related health including past diets, weight history, eating habits, exercise routines, sleep, and stress. We will define your nutrition-related goals and formulate a nutrition support plan to reach them.

We recognize that diabetes care is not one-size-fits-all, as every person is unique. For this very reason, we take the time to get to know our clients and their specific needs and goals to tailor our approach and help you make the most out of every appointment.

Understanding Your Diabetes

The very first step, and one of the most important, is understanding the current state of your health. During your initial assessment, we’ll review your health history, including any recent diagnostic tests you may have had, to paint a better picture of your health and diabetes.

We can better help you understand your health condition and how it’s affecting your life. This information, combined with other details like your current diet and exercise routine, can all come together to give us all the details we need to point you in the right direction.

Tailoring the Right Education Plan for You

Based on your health and needs, we can create an education and support plan to help reach your overall goals.

Our education plans vary, depending on whether you’re diagnosed with pre-diabetes, type 1 diabetes, or type 2 diabetes. Whatever your diagnosis, our team aims to create a plan that follows the seven pillars and self-care behaviors of diabetes management, which include the following.

Monitoring

Monitoring your blood sugar is key to understanding your diabetes and the impact of your meals, exercise, stress and even sleep.

Monitoring your blood sugar can help you make informed decisions about your food choices, timing of activity and more.

Regardless of your current monitoring schedule or method of monitoring our goal is to provide education and support.

Taking Medications

There are many options when considering medications for diabetes. These different classes of medications work in specific ways to lower blood sugar levels.

When choosing a medication to treat diabetes your doctor might consider if the medication is known to cause weight gain, does it increase the risk of hypoglycemia (low blood sugar), how it is administered as well as other considerations.

Working with a diabetes educator can help you understand how your medication works, how it should be taken and what to do if you miss a dose, etc.

Healthy Diet

There is no “one size fits all” diet for all people living with diabetes, however diet does have a significant impact on aspects of diabetes management including blood sugar, blood pressure and cholesterol levels.

With this in mind, research supports an individualized nutrition plan for each person living with diabetes with the goal of including a variety of nutrient dense foods, considering cultural and lifestyle factors, maintaining the pleasure of eating and supporting a practical and easy to maintain eating pattern.

Living with diabetes means you may have an increased risk of health related complications however making lifestyle changes however staying up to date on regular screenings and checkups and help reduce these risks.

Problem-Solving

Life happens and problems arise, especially when managing diabetes. However, learning how to take action and address problems as they arise is crucial.

We can work with you to develop a problem-solving plan so you always feel better prepared. For example, if your blood sugar plummets, we can give you specific instructions to tackle the issue and get your blood sugar up.
